H1N1 Flu Shots: We began
receiving shipments of the H1N1 vaccine in the
month of October, but supplies are currently limited. To expedite service when you attend
a clinic or come to the Health Department for your
immunization, please download, print and complete a copy of
the H1N1 Consent and History form for each person receiving
the vaccine before coming in for your immunization(s).

clinics, please download, print, and complete a copy of the
consent form for each member of your family before coming in
for your shot(s).
Cost of the seasonal flu shot is $25 -- free for seniors
with Medicare Part B.
